Brookstone Senior Living a Phoenix Senior Living Community seeking a Resident Care Director to join their team!RELATIONSHIPThe Resident Care Director reports directly to the Executive Director.PURPOSEThe Resident Care Director is responsible for providing overall leadership and management of the health and well-being of the residents within the community. Responsible for facilitating resident care in-services and the completion of Personalized Service Plans.Responsibilities include but are not limited to all clinical programs, quality assurance and regulatory compliance; assisting in the training and on-boarding of resident care associates and medication technicians.PRINCIPLE DUTIES AND RESPONSIBILITIESResident CareIdentifies on-going needs and services of residents through the assessment / Personalized Service Plan (PSP) processEnsures proper follow through and documentation for residents with a change in conditionCompletion of the Wellness Evaluation & Assessment and PSP at the time of move-in, semiannually (per state regulations) and with any change in statusCompletes monthly wellness visits and PSP to accurately reflect the resident on a monthly basisPartners with other department directors to identify, review, discuss potential changes in service / medication levelsInforms the Executive Director of any changes in medication or service level that may result in a higher tier level or potential move-outEnsures weights and vital signs are obtained monthly for each resident prior to completion of monthly wellness visitContacts resident's attending physician when necessary and or upon family requestMaintains Wellness files according to Phoenix policy, federal and state guidelinesMaintains wellness supplies for the community emergency kitsOversee the monitoring of each resident's medication and treatment profile to ensure each medication and treatment is administered as ordered and documented accuratelyAccurately and promptly transcribes physicians orders MAR/TAR (Medication Administration Record /Treatment Administration Record) to include initials, date and time noted and date faxed to pharmacyCompletes MAR/TAR audit each monthDemonstrates and knowledgeable of the Six (6) Rights of Medication Pass ("Right" resident, medication, dosage, time, route, right to refuseMonitors the Medication Room, med charts, treatment carts for neatness, cleanliness, availability of medications and expired medicationsTrainingPartners in the delivery and participation in Phoenix Academy training and self-study programs during the required time-frame.Completes All Phoenix Academy, job specific and management training in the timeframe designatedCompletes all training required by the state or other regulating authorities including ongoing training per regulationCommunicationEnsures residents and families are educated about residents' rightsCreates and maintains a warm professional environmentAssures that all staff is fully informed on the answers to the most frequently asked questions by residents, families, and prospective familiesFacilitates regularly scheduled meetings with department heads and associatesManages appropriate communications to Executive DirectorPossesses excellent communication skills, both verbal and written, and the ability to relate in a positive and professional way to a diverse resident and associate populationActs as liaison for ancillary support services to ensure visits are made as scheduled or prescribed and prompt communication between associates and ancillary service providerActs as a liaison for pharmacy services to ensure effective services for residentsCompliance/SafetyEnsures compliance with state assisted living regulationsSubmit all state reportable incidents to Clinical Support Specialist and VP of RiskEnsures compliance with all state and federal regulations (i.e. OSHA nurse practice acts, etc.) and Phoenix standards as it pertains to clinical careUnderstands and practices the proper method of attending to and disposing of and the possibility of exposure to blood borne pathogens, bodily fluids, infectious waste, sharp sticks and hazardous chemicals immediatelyOversees community risk management programResponsible for collecting, analyzing, and Reporting Phoenix clinical indicators monthly and if necessary, participate in the development and implementation of a community improvement planPartner with the Executive Director and other community associates to analyze and maintain all resident and associate safetyPartners with pharmacy consultant to provide optimal pharmaceutical services to residentsResponsible for infection control programmingResponsible for the safe and efficient distribution / administration of injections, nebulizers, medications and moderate wounds care per state / provincial regulationsActs as Lead in an emergency (e.g. elopement, fire, disaster, etc.) according to the chain of authority identified in the community disaster plan#MGM001#Brookstone#INDEEDPSL
