Overview: The Primary Care Assistant - LPN plays a crucial role in
assisting primary care providers by delivering patient-focused

Patient Care LPN collaborates directly with our primary care

will be instrumental in providing compassionate, patient-centered
care. Your responsibilities will include preparing patients for
appointments, performing clinical tasks such as taking vital signs,
reconciling medications, conducting in-office tests, and assisting
with procedures. LPNs in our organization help bridge care gaps,
administer vaccines, and ensure accurate documentation to foster

care, this opportunity is for you! Key Responsibilities: Facilitate
patient care and cultivate strong relationships in partnership with
primary care providers. Prepare and organize examination rooms,
ensuring all necessary equipment and supplies are available for
patient visits. Escort patients to examination rooms and prepare
them for treatment with professionalism and respect. Complete the
rooming process: document chief complaints, take vital signs,
review medical history and allergies, reconcile medications, and
address care gaps (e.g., screenings and preventive measures).
Assist with in-office testing, including blood draws, EKGs, oxygen
studies, mental status evaluations, and point-of-care testing.
Administer vaccines and injections within the scope of practice and
support providers during procedures. Input orders, schedule
follow-up appointments, and ensure accurate documentation in the
electronic health record system. Collaborate with providers and the
care team to monitor and coordinate care plans, ensuring an
exceptional patient experience. Perform additional tasks as
assigned to support clinical operations. Qualifications: Education:
Completion of an accredited practical nursing program. Licensure:
Must hold a valid LPN license in the state of practice. Preferred
Experience: At least 6 months of clinical healthcare experience,
ideally in a primary care setting. Other Skills and Abilities:
Ability to work effectively as part of a team and collaborate with
all members of the care team. Strong written and verbal
communication skills. Customer service-oriented with excellent
organizational and interpersonal skills. Flexible and positive
demeanor. Attention to detail and accuracy. Self-motivated and
capable of performing tasks independently within the scope of
practice with minimal supervision. Physical Requirements: Must be
able to push, pull, and/or lift up to 50 pounds regularly. Requires
prolonged standing and walking during each shift. Ability to grip,
reach, bend, kneel, twist, and squat to fulfill job duties. We
