Job Description:

The Care Management model provides effective transition planning

adoption of best practices. Qualifications: Preferred Experience:
2-3 years acute care hospital setting, preferred Required
Education: Graduate of an accredited School of Nursing, required
License: Current RN License in the state of employment required
Education : Bachelor's Degree of Science in Nursing, minimum
required Certifications: BLS (Basic Life Support) at hire date, or
