Job Description:
Agape Care Group is seeking a compassionate and dedicated Nurse
Practitioner to join our growing team of professionals providing
hospice and palliative care across the Southeast. The ideal
candidate will have a strong commitment to enhancing patient
quality of life in a supportive and dignified manner. As part of
Agape Care Group, you will engage closely with patients and their
families, implementing bespoke care plans aimed at managing pain
and symptoms in a peaceful environment. Conduct comprehensive
patient assessments to determine appropriate care needs and
interventions. Develop and implement care plans in collaboration
with patients, their families, and other medical professionals.
Manage symptom relief and pain control using evidence-based
practices and treatments. Provide guidance, education, and support
to patients and their families regarding end-of-life care options
and decisions. Monitor patient progress and make necessary
adjustments to the care plan. Prescribe medications as per the
state and federal regulations. Ensure compliance with healthcare
laws, regulations, and Agape Care Group’s internal protocols.
Proficiency in managing palliative care patients. Strong
organizational and communication skills. Ability to make critical
decisions independently in a high-pressure environment. Empathetic
and compassionate customer service orientation. Medical Insurance
Dental Insurance Vision Insurance Life Insurance 401k Current and
valid state Nurse Practitioner license. Certification in Hospice
and Palliative Care Nursing (CHPN) preferred.
