Job Description Job Description About the Role: The Home Care Nurse plays a critical role in delivering personalized nursing care to patients within their own homes, ensuring comfort, safety, and improved health outcomes. This position involves comprehensive assessment, planning, and implementation of nursing interventions tailored to each patient's unique medical and emotional needs. The nurse will collaborate closely with physicians, family members, and other healthcare professionals to coordinate care and monitor patient progress. By providing education and support to patients and their families, the Home Care Nurse empowers them to manage health conditions effectively and maintain independence. Minimum Qualifications: Valid and current Registered Nurse (RN) or LPN license in the state of practice. Basic Life Support (BLS) certification. Minimum of one year of clinical nursing experience. Ability to travel to patients' homes reliably and maintain a flexible schedule. Preferred Qualifications: Experience with electronic health records (EHR) and telehealth technologies. Sounds knowledge base of medications and adverse reactions Strong background in chronic disease management such as diabetes, heart failure, or COPD. Responsibilities: Conduct thorough patient assessments to evaluate health status, needs, and care plans. Skin Monitoring Medication set up and management Monitor patient progress and document changes in condition, reporting any concerns promptly. Educate patients and their families on disease management, medication adherence, and healthy lifestyle practices. Coordinate with interdisciplinary teams to ensure continuity and quality of care. Provide emotional support and counseling to patients and caregivers to address psychosocial challenges. Maintain accurate and timely medical records in compliance with regulatory standards. Ensure adherence to infection control protocols and safety guidelines during home visits. Skills: The Home Care Nurse utilizes clinical assessment and critical thinking skills daily to evaluate patient conditions and adjust care plans accordingly. Effective communication skills are essential for educating patients and families, as well as collaborating with healthcare teams to coordinate care.
