The Care Manager plays a vital role within the interdisciplinary
team, focusing on assessing, planning, advocating, and coordinating
patient care from the moment of admission through to discharge.
This position ensures a seamless transition following
hospitalization. Responsibilities are carried out in alignment with

Management model emphasizes effective transition planning and
oversight of length of stay, aiming to enhance patient experience,
safety, and quality of care by utilizing performance metrics and
implementing best practices. Position Overview: The RN Care Manager
II - IPCM is an integral member of the interdisciplinary team,
responsible for evaluating patients' health requirements, crafting
personalized care plans, and coordinating services to achieve
optimal health outcomes. This role necessitates effective
communication with patients, families, and healthcare
professionals. Advocating for patients' needs, promoting health
education, and ensuring smooth transitions across various
healthcare settings are key aspects of this position. Clinical
expertise, critical thinking abilities, and a compassionate
approach are essential for enhancing patient well-being and
improving healthcare results. Responsibilities are performed in

standards. Qualifications: Education: Bachelor’s Degree in Nursing
is required. Licensure: A current RN License in the state of
employment is mandatory. Experience: At least 1 year of experience
in an acute care hospital setting is required. Required
Certifications: BLS (Basic Life Support) certification must be
obtained at the time of hire or within 90 days (approximately 3
months) of employment. Preferred Certification: Certification in
